SLS Referral Form
To receive tutoring services, the Student Learning Services tutoring referral form must be completed and submitted to the Office of Student Learning Services. The top portion of the referral form should be completed and signed by the student; the bottom portion of the form should be completed and signed by the instructor.
In addition to being available for online download in PDF format (above), these forms are also available in the school lobby, the SLS Department, the Department of Student Affairs, or from the instructor.
